Bless the LORD, O ________’s soul;

And all that is within ________, bless His holy name!

Bless the LORD, O ________’s soul,

And forget not all His benefits:

Who forgives all ________’s iniquities,

Who heals all ________’s diseases,

Who redeems ________’s life from destruction,

Who crowns ________ with lovingkindness and tender mercies,

Who satisfies ________’s mouth with good things,

So that ________’s youth is renewed like the eagle’s.
